If you are a student, teacher, or adult supervisor interested in creating an IMRiversJr account please fill out and send the form below with the information listed below. If your project meets the IMRivers Junior criteria, then you will be provided with your own mapping site free of charge.

Please provide: :

- Name and position (student, teacher, counselor, etc.)

- School or sponsoring organization

- City, State, and Zip Code of organization

- Adult in charge of the proposed project

- Adult’s position in you school/other org. and contact information (phone and email)

- Topic of project ( a brief outline of what you want to do and how you plan on doing it)

- Timetable (When do you want to start? How long will the project run?)

Upon completion of your IMRivers Junior project, you and/or your class will receive a certificate acknowledging your hard work and commitment to improving the health of your community.
